Pharmacological properties

Ambroxol (trans-4-[(2-amino-3,5-dibromobenzyl)amino]cyclohexanol hydrochloride) exhibits a pronounced mucolytic and secretomotor effect. Stimulates serous cells of the glands of the bronchial mucosa, thereby changing the disturbed ratio of serous and mucous components of sputum. At the same time, hydrolyzing enzymes are activated and the release of lysosomes from Clark cells is enhanced, due to which the viscosity of sputum is reduced. Ambroxol enhances the movement of cilia of the ciliated epithelium of the bronchi, increasing mucociliary clearance. The drug also stimulates the formation of surfactant.

After taking the drug, ambroxol is rapidly and almost completely absorbed from the digestive tract. Ambroxol undergoes the effect of first-pass metabolism through the liver, bioavailability is 70-80%. The maximum concentration in the blood serum is reached 2 hours after oral administration. Binding to plasma proteins is approximately 84%. Ambroxol penetrates the blood-brain barrier and the placental barrier, is excreted in breast milk. It is metabolized to form pharmacologically inactive metabolites, which are excreted in the urine (approximately 90%). The half-life is 7-12 hours.

Indication

Acute and chronic diseases of the respiratory tract, accompanied by the secretion of viscous sputum - bronchitis, bronchiectasis, pneumonia.

Application

Inside after meals. In the first 2-3 days, adults are prescribed 10 ml (2 teaspoons) 3 times a day, then 5 ml (1 teaspoon) of syrup 3 times a day. Children aged 2 to 6 years - 2.5 ml (1/2 teaspoon) 3 times a day; over 6 years - 5 ml (1 teaspoon) 2-3 times a day. The course of treatment, depending on the severity of the disease, is 4-14 days.

Contraindication

I trimester of pregnancy, breastfeeding period, age up to 2 years (the drug contains alcohol), gastric and duodenal ulcers, hypersensitivity to the components of the drug.

Side effects

Allergic reactions (skin rash, urticaria, angioedema), with prolonged use in high doses - pain in the epigastric region, nausea, vomiting.

Special instructions

The drug does not contain sugar and can be used by diabetics.

Interactions

Enhances the effect of ampicillin and amoxicillin by increasing their concentration in the lung tissue. Therapeutically, the combination of ambroxol-health with theophylline is advisable, its joint use with antibiotics, cardiac glycosides, diuretics is possible.

Overdose

Epigastric pain, nausea, vomiting may occur. Treatment is symptomatic.
