Carbetocin solution for injection is indicated for the prevention of uterine atony in the event of cesarean section performed using spinal or epidural anesthesia.
Composition
Active ingredient: carbetocin;
1 ml of solution contains 100 mcg of carbetocin;
Excipients: sodium chloride, glacial acetic acid, water for injections.
Contraindication
Hypersensitivity to carbetocin or oxytocin. Pregnancy and childbirth before childbirth. Should not be used to stimulate labor. Liver and kidney disease. Episodes of preeclampsia and eclampsia. Severe cardiovascular diseases. Epilepsy.Method of application
Carbetocin should only be administered under appropriate medical supervision in a hospital setting.
The drug is administered in a dose of 1 ml once slowly, within 1 minute, only after cesarean section and childbirth. Carbetocin should be administered immediately after childbirth, preferably before the placenta is separated. The drug should not be administered further.

Overdose
Exceeding the dose of carbetocin may cause increased uterine activity both in the presence and absence of hypersensitivity to this drug.
Hyperactivity accompanied by strong (tonic) or prolonged contractions caused by carbetocin overdose may lead to uterine rupture and postpartum hemorrhage.
In severe cases, overdose of oxytocin can lead to hyponatremia and hyperhydration, especially in connection with the simultaneous administration of excessive amounts of fluid. Since carbetocin is an analogue of oxytocin, the possibility of developing similar effects cannot be excluded.
Treatment: symptomatic and supportive therapy. If symptoms of overdose occur, the woman in labor should begin oxygen therapy. In case of hyperhydration, it is essential to limit the amount of fluid, stimulate diuresis, correct electrolyte imbalance, and stop convulsive syndrome if it occurs.
Side effects
Blood and lymphatic system disorders: anemia. Gastrointestinal disorders: nausea, abdominal pain, metallic taste in the mouth, vomiting. General disorders and administration site conditions: feeling of heat, chills, pain. Musculoskeletal system disorders: back pain. Nervous system disorders: headache, tremor, dizziness. Respiratory system disorders: chest pain, shortness of breath. Skin and subcutaneous tissue disorders: itching. Vascular disorders: hypotension, flushing.Interaction
Severe hypertension was observed after oxytocin was administered 3 to 4 hours after prophylactic administration of vasoconstrictors for spinal anesthesia.
Oxytocin and carbetocin, when used together with ergot alkaloids such as methylergometrine, may increase blood pressure, enhancing the effects of these drugs. The risk of cumulative effects is increased if oxytocin or methylergometrine is administered after carbetocin.
