Pharmacological properties
Sodium salt of 2-(2,6-dichlorophenylamino)-phenylacetic acid, nonsteroidal anti-inflammatory drugs. has anti-inflammatory and analgesic effects due to inhibition of the biosynthesis of prostaglandins, kinins and other mediators of inflammation and pain, reducing capillary permeability and stabilizing effect on lysosomal membranes. during course treatment of diseases of the musculoskeletal system, the drug intensively penetrates the joint cavities, contributing to the reduction of pain in the joints at rest and during movement, increasing the volume of movements, reducing morning stiffness and swelling of the joints. A stable therapeutic effect is observed 1-2 weeks after the start of treatment.
When administered rectally, diclofenac is rapidly absorbed by the rectal mucosa. The maximum concentration in the blood plasma is reached after 1-2 hours. The half-life is on average 2 hours. About 70% of the administered amount of the drug is excreted from the body with urine, the rest with feces.
Indication
Rheumatoid arthritis, ankylosing spondylitis (Bechterew's disease), acute gouty, infectious, psoriatic and traumatic arthritis, osteoarthritis, spondyloarthritis, primary dysmenorrhea, adnexitis, other inflammatory and degenerative-inflammatory diseases of the musculoskeletal system, accompanied by pain syndrome.
Application
The dose of the drug depends on the nature, severity and course of the disease. Usually, adults are administered rectally 1 suppository 2 times a day for 10-15 days.
Contraindication
Hypersensitivity to diclofenac, erosive-ulcerative lesions of the digestive tract (gastric and duodenal ulcer, nonspecific ulcerative colitis, Crohn's disease), severe renal or hepatic insufficiency, hematopoietic disorders, pregnancy and breastfeeding.
Side effects
Usually well tolerated. In some patients with hypersensitivity to diclofenac, symptoms of local irritation of the rectal mucosa (itching, burning), constipation, flatulence may occur. In case of exceeding the recommended doses, systemic side effects characteristic of diclofenac and other nonsteroidal anti-inflammatory drugs (nausea, belching, discomfort in the epigastric region, diarrhea, headache, dizziness) are possible, which require a reduction in the dose of the drug.
Special instructions
It is prescribed with caution to patients who have a history of indications for BA and allergic reactions to nonsteroidal anti-inflammatory drugs.
Interactions
Rectal suppositories with diclofenac sodium are not prescribed together with other NSAIDs and corticosteroids, given the possibility of increased side effects. The therapeutic efficacy of diclofenac when used simultaneously with salicylates decreases, while the ulcerogenic effect of the latter on the gastric mucosa increases. Diclofenac is not recommended for patients who use digoxin or lithium preparations (due to the risk of increasing the concentrations of these drugs in the blood plasma). With caution, under careful monitoring of the parameters of the blood coagulation system, diclofenac can be prescribed together with heparin and coumarin anticoagulants. It should be borne in mind that the drug is able to weaken the effect of thiazide diuretics and furosemide.
