Diclofenac-Zdorovye Ultra gel for external use 5% tube 50 g

Pharmacological properties
Anti-inflammatory, analgesic, antirheumatic agent. The mechanism of action is due to the inhibition of prostaglandin biosynthesis by inhibiting COX, reducing the formation of kinins and other mediators of inflammation and pain, stabilizing the effect on lysosomal membranes. In rheumatic diseases, it reduces edema, hyperemia and pain at rest and during movement, morning stiffness and swelling of the joints, improving their functional ability. In postoperative and traumatic injuries, it quickly reduces pain and inflammation in the area of application of the drug, promotes the resorption of post-traumatic hematoma.
Dimethyl sulfoxide and essential mint oil, which are part of the Diclofenac-Zdorovya Forte gel, enhance the anti-inflammatory and analgesic effects of the drug.
Diclofenac when applied externally penetrates well through the skin, concentrating mainly in the focus of inflammation and synovial fluid. Binding to blood plasma proteins is 99%. Metabolized in the liver. Excreted by the kidneys (about 65%) and with bile (about 35%) in the form of inactive compounds with glucuronic and sulfuric acids; less than 1% is excreted unchanged.
Indication
Rheumatoid arthritis, ankylosing spondylitis, acute gouty, infectious, psoriatic and traumatic arthritis, osteoarthritis, bursitis, tendovaginitis, radiculitis, thrombophlebitis, neuralgia, myalgia; symptomatic treatment of diseases accompanied by inflammation and pain.
Application
Diclofenac-health ultra is used externally.
Spray.
The required amount of the drug should be applied to the skin at the site of the lesion. Depending on the size of the treated skin surface, it is necessary to make 2-3 presses on the sprayer (0.8-1.2 ml of spray, which corresponds to 32-48 mg of diclofenac sodium). The drug should be applied 3 times a day at equal intervals.
A single dose of 1.2 ml (3 sprays) of the drug should not be exceeded. The maximum daily dose is 9 sprays (3.6 ml of spray, corresponding to 144 mg of diclofenac sodium).
The drug should be gently rubbed into the skin, after which hands should be washed thoroughly, except when they are the site of treatment.
Gel.
Adults and children over 12 years of age apply the gel in a thin layer 3-4 times a day, lightly rubbing into the skin.
The amount of the drug depends on the size of the affected area (for example, 2-4 g of gel, the volume of which corresponds to the size of a cherry or walnut, is enough to apply to an area of 400-800 cm2).
After applying the drug, hands should be washed.
The duration of the treatment course is determined individually depending on the indications and effectiveness of therapy, which should be determined no less than 2 weeks after the start of treatment.
The drug should not be used for more than 14 consecutive days for soft tissue pathology or more than 21 days for arthritis.
Elderly patients: no dose adjustment is required.
Contraindication
Erosive-ulcerative lesions of the gastrointestinal tract, ba, hematopoietic disorders, hypersensitivity to the components of the drug, pregnancy and breastfeeding, children under 14 years of age.
Side effects
Possible local skin irritation: itching, hyperemia, burning, rash.
Special instructions
The drug should be applied only to intact skin, avoiding contact with damaged surfaces, open wounds, eyes and mucous membranes. After applying the drug, wash your hands if the area is not to be treated.
The use of the gel can be combined with the intake of diclofenac sodium tablets in a daily dose not exceeding 75 mg.
The gel is prescribed with caution to patients with gastric and duodenal ulcers, bronchial asthma, rhinitis, nasal mucosa polyps, with impaired liver, kidney, and hematopoietic function, and with simultaneous use of other nonsteroidal anti-inflammatory drugs.
Interactions
Interaction with other drugs when using the gel externally has not been established. However, it should be borne in mind that diclofenac sodium can potentiate specific effects, including ulcerogenic effect of GCS, increase the concentration of digoxin and lithium in the blood, weaken the effect of thiazide diuretics and furosemide. Therapeutic efficacy when used with salicylates decreases, while the ulcerogenic effect on the gastric mucosa increases. When used simultaneously with anticoagulants, the state of the blood coagulation system should be monitored.
Overdose
When applying the gel to large areas of skin or when exceeding the recommended doses, systemic side effects typical of diclofenac sodium and other nonsteroidal anti-inflammatory drugs are possible: dyspepsia, nausea, epigastric pain, headache, dizziness, drowsiness, bronchospasm.
Treatment: dose reduction or discontinuation of the drug, symptomatic therapy.
