Pharmacological properties
The drug has anti-inflammatory, analgesic, angioprotective, antithrombotic effects. The mechanism of action of diclofenac is due to inhibition of the biosynthesis of prostaglandins, kinins and other mediators of inflammation and pain, a decrease in capillary permeability and a stabilizing effect on lysosomal membranes. Nicotinic acid improves microcirculation, exhibits antithrombotic effects, and promotes the activation of redox processes in tissues.
The pharmacokinetics of the drug is due to the complex action of the components. When used in the form of a gel, diclofenac is very slowly, little and insignificantly (3-6% of the dose applied to the skin) absorbed into the skin. With prolonged treatment or application to large areas, it gradually penetrates into soft tissues, synovial fluid, and enters the blood plasma. The concentration in the synovial fluid is quite stable, in the blood plasma it quickly decreases. Diclofenac is metabolized in the liver by conjugation with glucuronic acid. Approximately 70% is excreted in the urine, 35% in the feces. Nicotinic acid has a local effect.
Indication
Acute and chronic venous insufficiency of the lower extremities, post-thrombophlebitic syndrome, as well as rheumatoid arthritis, ankylosing spondylitis, acute gouty, infectious, psoriatic and post-traumatic arthritis, osteoarthritis, bursitis, tendovaginitis, radiculitis, edematous and painful syndrome of traumatic etiology (strokes, sprains).
Application
The drug is applied 1-2 g (4-8 cm) 2-4 times a day in a uniform thin layer on the affected area, lightly rubbing into the skin. The course of treatment is 7-14 days.
Contraindication
Hypersensitivity to the components of the drug. gastric and duodenal ulcers, epilepsy, parkinsonism, mental illnesses, pregnancy and breastfeeding; not prescribed to young children.
Side effects
Some patients may experience local skin irritation - rash, itching, hyperemia.
Special instructions
It is prescribed with caution to patients with a history of liver, kidney, gastrointestinal tract diseases. Avoid contact of the gel with wounds, as well as with eyes and mucous membranes. In order to increase the effectiveness of therapy, it is possible to use occlusive dressings.
Interactions
The interaction of Diclosan with other drugs has not been established. It should be borne in mind that Diclofenac can enhance the effect of anticoagulants, glucocorticoids, lithium preparations, as well as weaken the effect of furosemide and thiazides, increase the concentration of digoxin in the blood plasma, reduce the tolerance of other nonsteroidal anti-inflammatory drugs, increasing their ulcerogenic effect on the mucous membrane of the gastrointestinal tract.
Overdose
Possible general resorptive side effects characteristic of diclofenac - vomiting, belching, epigastric discomfort, diarrhea, headache, dizziness. Symptomatic therapy is recommended, there is no specific antidote.
Storage conditions
In a place protected from light at a temperature of 10-20 °C.
