Pharmacological properties
A cardiac glycoside contained in the leaves of foxglove. It has a pronounced positive inotropic effect, increases systolic and stroke volume of the heart, prolongs the refractory period, slows AV conduction and reduces heart rate. It has a moderately pronounced diuretic effect, is quickly excreted from the body, and cumulation is less pronounced than with digitoxin.
Indication
Congestive heart failure stage I-III; preparation of patients with heart disease for surgical interventions or childbirth; paroxysmal supraventricular tachyarrhythmia (tachyarrhythmic form of atrial fibrillation, atrial flutter, supraventricular paroxysmal tachycardia).
Application
Inside on the 1st day, 0.25 mg 4-5 times a day, on the following days - 0.25 mg 1-3 times a day (maintenance doses). i / v digoxin is administered at 0.25-0.5 mg (1-2 ml of 0.025% solution 1-2 times a day). The contents of the ampoule are diluted in 10 ml of 5% or 10% glucose solution or 0.9% sodium chloride solution and injected jet for at least 5 minutes.
Contraindication
Severe bradycardia, complete AV block, increased frequency of angina attacks, recent myocardial infarction.
Side effects
Possible slowing of heart conduction (blockade), bradycardia, extrasystole, lack of appetite, nausea, diarrhea; in children, in addition, deterioration of well-being, lethargy, anorexia. If arrhythmia and other side effects develop, the drug should be discontinued.
