Composition

Pharmacological properties
Pharmacodynamics.
Dimethyl sulfoxide has the property of penetrating biological membranes, including skin barriers, thus realizing its specific effects, which include anti-inflammatory, antipyretic, analgesic, antiseptic, moderate fibrinolytic. The drug enhances the penetration of drugs through intact skin and mucous membranes (transport ability).
Pharmacokinetics.
After application of a solution (90%) of dimexide to the skin, it is detected in the blood after 5 minutes, reaching a maximum concentration after 4-6 hours, with an almost constant level remaining for 1.5-3 days. Dimethyl sulfoxide is excreted in urine and feces both in unchanged form and in the form of dimethyl sulfone.
Indication
As part of the complex therapy of inflammatory diseases of the musculoskeletal system: rheumatoid arthritis, Bechterew's disease, deforming osteoarthritis (in the presence of periarticular tissue damage); in arthropathy, tendon sprains during impact, in traumatic infiltrates, acute and chronic osteomyelitis, streptoderma, purulent wounds, abscesses.
It is also used in skin plastic surgery - for the engraftment of transplanted skin auto- and homografts, as well as for the preservation of skin homografts.
Contraindication
Hypersensitivity to the drug. Cardiovascular failure, severe atherosclerosis, angina pectoris, myocardial infarction, impaired renal or hepatic function, stroke, comatose states, glaucoma, cataracts.
Interaction with other medicinal products and other types of interactions
Dimethyl sulfoxide enhances the effect of ethanol (alcohol inhibits the excretion of the drug) and insulin, acetylsalicylic acid, butadione; digitalis preparations, quinidine, nitroglycerin, antibiotics (streptomycin, monomycin, etc.), chloramphenicol, rifampicin, griseofulvin, increases the body's sensitivity to anesthetics. It should be taken into account that the drug enhances both the specific activity and toxicity of drugs. It can be used together with nonsteroidal anti-inflammatory drugs in the complex therapy of deforming osteoarthritis and rheumatoid arthritis; in combination with local antimicrobial agents (synthomycin liniment) - for the treatment of streptoderma.
Application features
Do not apply to damaged skin or skin with allergic manifestations. Do not apply to facial skin.
Before using the drug, it is necessary to conduct a sensitivity test. To do this, apply a small amount of the solution to the skin using a cotton swab. Sharp redness and itching of the skin after applying Dimexide indicate individual hypersensitivity to the drug.
Elderly patients can use the drug after prior consultation with a doctor.
Avoid contact with mucous membranes and eyes. If the product accidentally gets into the eyes, rinse them immediately with running water.
At temperatures below 18°C, dimethyl sulfoxide may crystallize, which does not affect the quality of the product. To melt the crystals, carefully heat the vial with the product in a water bath (water temperature about 60°C).

Use during pregnancy or breastfeeding
The drug is contraindicated for use during pregnancy. Breastfeeding should be discontinued during treatment with dimethyl sulfoxide.
Ability to influence reaction speed when driving vehicles or other mechanisms
It should be used with caution, considering the effects on the central nervous system, in particular the possibility of dizziness.
Method of administration and doses
It is used in the form of aqueous solutions (30-50% concentration) for compresses and tampons.
Gauze wipes moistened with a solution of Dimexide are applied to the damaged areas, capturing the adjacent healthy areas of the skin, once a day for 20-30 minutes. A plastic film and cotton or linen cloth are applied over the wipes.
For diffuse streptoderma, compresses with a 40-90% solution of Dimexide are recommended.
A 40% solution is used for pustular skin diseases.
In skin plastic surgery, dressings with a 10-20% solution of the drug are applied to transplanted skin auto- and homografts immediately after operations and in the following days of the postoperative period until the graft is stable.
A 5% solution of the drug in Ringer's solution is used as a preservative for storing skin homotransplants.

Children
Do not use the drug in children under 12 years of age.
Overdose
Overdose of the drug may cause allergic reactions (urticaria, edema) and increased side effects, which requires discontinuation of the drug and symptomatic therapy.
Adverse reactions
Dimethyl sulfoxide is usually well tolerated, but some patients may experience burning, skin hyperemia, erythema, dry skin, dizziness, headache, insomnia, adynamia, nausea, vomiting, diarrhea, hypersensitivity reactions, including dermatitis, skin rashes, angioedema, itching, bronchospasm.
