Pharmacological properties
A remedy that replenishes potassium deficiency. Helps maintain proper intracellular and extracellular levels of potassium, which is the main intracellular ion that plays an important role in cellular metabolism and the regulation of various body functions. Potassium ions activate many cytoplasmic enzymes, regulate intracellular osmotic pressure, increase the tone of the sympathetic division of the autonomic nervous system, increase the production of adrenaline by the adrenal glands, stimulate the synthesis of ATP, glycogen, proteins, acetylcholine. Reduce heart rate, reduce contractile activity, conductivity, automatism and excitability of the myocardium. In low doses, potassium dilates coronary vessels, in high doses it constricts them. Glucose is an easily digestible energy substrate and promotes the penetration of potassium into cardiomyocytes, increasing its intracellular concentration.
The drug eliminates electrolyte imbalance in the body, improves metabolism in the myocardium and its energy supply, has antiarrhythmic properties. Reduces increased sensitivity to cardiac glycosides and reduces their cardiotoxic effect.
When administered by infusion, the drug enters the cells, particularly cardiomyocytes, from the blood in the form of potassium ions. Potassium and glucose are involved in cellular metabolism. It is excreted in the urine.
Indication
Arrhythmia caused by hypokalemia, including in case of intoxication with cardiac glycosides. Severe hypokalemia (blood plasma potassium concentration less than 2.5 mmol/l).
Application
The dosage regimen is set individually under the control of ECG and potassium levels in the blood plasma. As a rule, adults are prescribed 1-2 infusions per day of 200 ml (1 g in terms of potassium chloride). The rate of administration is 20-30 drops per minute.
Contraindication
AKI, chronic renal failure, complete heart block, hyperkalemia, metabolic disorders (acidosis, hypovolemia with hyponatremia), hyperhydration, diabetes mellitus, glucose metabolism disorders, pregnancy and breastfeeding.
Side effects
Rarely and when used in high doses - weakness, paresthesia, paradoxical reaction in the form of an increase in the number of extrasystoles.
Special instructions
It is prescribed with caution in case of AV conduction disorders. It should be taken into account that the toxicity of potassium salts increases in case of adrenal insufficiency.
Interactions
The drug improves the tolerance of cardiac glycosides, which allows, if necessary, to simultaneously administer strophanthin or digitalis preparations. In case of heart rhythm disturbances caused by hypokalemia, insulin can be added to the infusion solution of the drug to increase the repolarization of heart cells. When used simultaneously with potassium-sparing diuretics and ACE inhibitors, the level of potassium in the blood should be monitored.
Overdose
It remains asymptomatic for a long time until the concentration of potassium in the blood plasma reaches a high level (6.5-8 mmol / l). Symptoms: abdominal pain, apathy, muscle weakness, paresthesias, an increase in the amplitude of the T wave, ST segment depression, QRS complex expansion are recorded on the ECG. Treatment: intravenous administration of calcium preparations, symptomatic therapy aimed at supporting vital functions, hemodialysis.
