Pharmacological properties
Normotymicheskoe drug. Lithium ions change the transport of sodium ions in neurons, increase intracellular deamination of noradrenaline, which reduces its free amount, increase the sensitivity of neurons to the action of dopamine. Glutamate in significant quantities is found in brain tissue and participates in the processes of reamination and binding of ammonia, which significantly increases the therapeutic effect of lithium ions. As a result, the drug has a pronounced normotymicheskoe effect.
It is well absorbed from the small intestine and evenly distributed in the body. Penetrates the blood-brain barrier and placenta. It is detected in breast milk. It is excreted by the kidneys by filtration. Approximately 4/5 of it is reabsorbed. The half-life is approximately 24 hours. With a decrease in the intake of sodium ions into the body, an increase in the half-life of lithium is observed.
Indication
Manic-depressive psychosis, periodic forms of schizophrenia, depression in old age, treatment of affective disorders in patients with chronic alcoholism.
Application
Adults are recommended to be prescribed in the initial and maintenance dose of 1 capsule 2 times a day in the morning and evening after meals, washed down with 1/2 glass of water. In order to enhance the therapeutic effect, the dose can be increased; the maximum daily dose is 2.1 g. After achieving the therapeutic effect, the dose is reduced to the maintenance dose. Periodic determination of the concentration of lithium in the blood plasma is mandatory; the maximum permissible concentration is 1.6 mol / l.
Contraindication
Impaired renal excretory function, cardiovascular diseases in the decompensation phase and with myocardial conduction disorders, pregnancy and breastfeeding, thyroid dysfunction.
Side effects
Usually well tolerated, in some cases a slight headache and nausea are possible. When taken in toxic doses, drowsiness, diarrhea, vomiting, dizziness, tremor, ataxia, blurred vision, tinnitus may occur.
Special instructions
Glutalith treatment should be carried out under the control of serum lithium concentration and provided that the excretory function of the liver and kidneys is normal. The drug should be used with caution in epilepsy and in case of sodium and water deficiency in the body (use of diuretics, intense sweating, diarrhea, hyponatremia).
Interactions
Concomitant use of glutalite with diuretics or nonsteroidal anti-inflammatory drugs may lead to an increase in serum lithium concentrations.
Overdose
In acute or chronic lithium poisoning, sodium bicarbonate and forced diuresis using euphylline, acetazolamide, etc., hemo- and peritoneal dialysis are prescribed. If the maximum permissible concentration of lithium in the blood (1.6 mol / l) is exceeded, muscle weakness, tremor, polyuria, thirst, and heart rhythm disturbances may occur.
