Pressure measurement is carried out auscultatively, i.e. by listening to Korotkoff tones.
These tones occur when the pressure in the cuff is compared to the pressure in the brachial artery.
Systolic pressure, or the upper pressure, is the point at which the heart makes a "knock" and is heard first. And its disappearance or sharp weakening, at which the pressure in the cuff becomes lower than the pressure of the blood on the artery wall, at the moment when the heart valves close (diastolic pressure, the lower pressure).
