Nitroglycerin concentrate for solution for infusion should be used for the following indications:
Severe and prolonged ischemic pain in the heart area associated with myocardial infarction or unstable angina. Insufficiency of cardiac pumping function and pulmonary edema associated with acute myocardial infarction. Arterial hypertension associated with open heart surgery and other surgical interventions. Ensuring controlled hypotension during surgical interventions.Composition
Active ingredient: nitroglycerin;
1 ml of glycerol trinitrate solution 5% in ethanol, calculated on 100% content of the main substance - 10.0 mg;
Excipients: ethanol (96%).
Contraindication
Hypersensitivity to nitroglycerin and other nitro compounds. Hypovolemia, severe arterial hypotension (systolic blood pressure <90 mm Hg), acute myocardial infarction localized in the right ventricle, increased intracranial pressure (due to head injury or cerebral hemorrhage), cardiac tamponade, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, angle-closure glaucoma, toxic pulmonary edema, anemia, acute vascular insufficiency (shock, vascular collapse).
Sildenafil. Simultaneous use of the phosphodiesterase type 5 inhibitor sildenafil, since sildenafil potentiates the hypotensive effect of nitrates (the interval between doses should be at least 48 hours).
Method of application
The dosage regimen is set individually, depending on the clinical response and systolic blood pressure.
An infusion solution containing 100 mcg/ml nitroglycerin is usually used.

Overdose
Symptoms: hypotension, tachycardia, feeling of heat, flushing, headache, palpitations, fainting. Increased intracranial pressure, which can lead to confusion and neurological disorders. Overdose within a few hours can lead to ethanol intoxication.
Treatment. Excessive hypotension can be eliminated by reducing the infusion rate of the drug or stopping its administration. In case of severe arterial hypotension, the patient should be placed in a horizontal position with the head of the bed lowered. In case of hypotension associated with bradycardia, the use of atropine and dopamine is indicated. Hypoxia caused by methemoglobinemia can cause cyanosis, metabolic acidosis, coma, convulsions, vascular collapse. In case of methemoglobinemia, a solution of methylene blue (1-2 mg/kg body weight) is administered.
Side effects
In most cases, the side effects of nitroglycerin are associated with an excessive decrease in blood pressure.
Interaction
Sildenafil. Concomitant use of the phosphodiesterase type 5 inhibitor sildenafil is contraindicated, as sildenafil potentiates the hypotensive effect of nitrates (the interval between doses should be at least 48 hours).
