Novostezin is a drug for local anesthesia by percutaneous infiltration, peripheral nerve block(s) and central neural block (caudal or epidural), i.e. it is used by a specialist in situations where prolonged anesthesia is required. Since sensory nerve block is more pronounced than motor block, bupivacaine is particularly effective in relieving pain, for example during childbirth.
Warehouse
1 ml of solution contains bupivacaine hydrochloride in terms of anhydrous substance 2.5 mg; excipients: sodium chloride, sodium hydroxide or concentrated hydrochloric acid, water for injection.Contraindication
Hypersensitivity to any of the components of the drug or to local anesthetics of the amide group; intravenous regional anesthesia (Bier's block), because accidental penetration of bupivacaine into the bloodstream can cause the development of acute systemic toxic reactions; epidural anesthesia in patients with severe arterial hypotension, for example, in the case of cardiogenic or hypovolemic shock.Method of application
The dosage should be adjusted depending on the degree of blockade and the general condition of the patient.
For infiltration anesthesia, 5-30 ml of a 5 mg/ml solution of the drug (25-150 mg of bupivacaine hydrochloride) should be administered.
For intercostal block, 2-3 ml of a 5 mg/ml solution of the drug (10-15 mg bupivacaine hydrochloride) should be administered per nerve for a total of 10 nerves.
For large nerve blocks (e.g. epidural, sacral and brachial plexus anesthesia), 15-30 ml of a 5 mg/ml solution of the drug (75-150 mg bupivacaine hydrochloride) should be administered.
For obstetric anesthesia (e.g. epidural anesthesia and caudal anesthesia for vaginal delivery or vacuum extraction), 6-10 ml of a 5 mg/ml solution (30-50 mg bupivacaine hydrochloride) should be administered. The doses given are initial doses and may be repeated every 2-3 hours if necessary.
For epidural block (when performing a cesarean section), 15-30 ml of a 5 mg/ml solution of the drug (75-150 mg of bupivacaine hydrochloride) should be administered.
Application features
Procedures using regional or local anesthetics, except for the simplest ones, should always be performed with the equipment necessary for resuscitation. Intravenous catheters should be placed before the local anesthetic is administered for major blocks.
Pregnant women
There is no evidence of adverse effects on human pregnancy, but the drug should not be used in early pregnancy unless the benefits are considered to outweigh the risks.
Children
Do not use in this category of patients.
Drivers
Depending on the dose and method of administration, bupivacaine may have a temporary effect on movement and coordination.
Overdose
Accidental intravascular injections of local anesthetics can cause immediate (seconds to minutes) systemic toxic reactions. In case of overdose, systemic toxicity occurs later (15-60 minutes after injection) due to the slower increase in blood concentrations of the local anesthetic.
Side effects
On the part of the immune system: allergic reactions, anaphylactic shock.
Nervous system: paresthesia, dizziness.
From the organs of vision: double vision.
Cardiac: bradycardia, cardiac arrest, cardiac arrhythmias.
Vascular disorders: arterial hypotension, arterial hypertension.
Storage conditions
Store in the original packaging at a temperature not exceeding 25°C. Do not freeze. Keep out of the reach of children.
