Composition
active ingredient: permethrin;
1 g of ointment contains 40 mg of permethrin;
excipients: ethanol 96%, carbomer 980, triethanolamine, purified water.

Pharmacodynamics
Permethrin is a mixture of cis- and trans-isomers of a synthetic pyrethroid. It is an insecticidal and acaricidal agent for external use, which acts on many insects, including scabies mites. It inhibits the development and causes the death of larvae and sexually mature individuals. The antiparasitic effect is due to the violation of ion permeability through sodium channels, inhibition of the polarization (repolarization) processes of the insect nerve cell membrane, which causes a paralyzing effect. Permethrin is low-toxic to humans and warm-blooded animals.
Pharmacokinetics
After cutaneous application, absorption is no more than 2%. Part of the substance that enters the systemic bloodstream is rapidly hydrolyzed to inactive metabolites, excreted mainly in the urine. As an anti-scabies agent, it is effective after a single procedure.
Indication
Scabies.
Contraindication
Hypersensitivity to the active substance, other pyrethroids or pyrethrins or to any of the excipients, acute inflammatory diseases of the scalp.
Interaction with other medicinal products and other types of interactions
Corticosteroids used to treat eczematous reactions should be discontinued before applying permethrin ointment, as there is a risk of exacerbation of scabies symptoms due to decreased immune function. There is a possibility that this interaction will lead to increased adverse reactions or reduced efficacy of both drugs.
Application features
Permethrin ointment 4% is for external use only.
It is necessary to avoid getting the ointment on the conjunctiva of the eyes. If this happens, the mucous membranes should be washed with plenty of running water. Personnel who carry out treatment with the drug should protect their hands with rubber gloves. After the procedure, wash your hands thoroughly with soap.
For the treatment of children, the drug should be used only as prescribed by a doctor.
If itching does not go away within 4 weeks after scabies treatment, the patient should consult a doctor.
To avoid re-infection with scabies, it is necessary to treat (thermally) clothing, hats, and bed linen.
In the presence of skin diseases, impaired skin integrity, inflammatory processes and secondary skin infections, as well as when using other medications, you should consult a doctor.
Use during pregnancy or breastfeeding
Limited data on the use of permethrin ointment during pregnancy, after cutaneous application, absorption is no more than 2%. The drug is not recommended for use during pregnancy, except in cases where the expected benefit to the mother outweighs the potential risk to the fetus and child. Breastfeeding should be discontinued for the period of treatment.
Ability to influence reaction speed when driving vehicles or other mechanisms
There are no reports of the drug's effect on the ability to drive or operate other mechanisms.
Method of administration and doses
Apply a small amount of ointment in a thin layer to the skin areas affected by scabies and rub it thoroughly into the skin with massaging movements. 24 hours after the procedure, the ointment must be washed off with soap and water and clothes changed. As a rule, the procedure should be carried out once, but if the itching does not stop or new scabies elements appear (a sign of insufficient effectiveness), the treatment should be repeated after 14 days (not earlier).
Children
Contraindicated for use in children under 2 months. For children over 2 months of age, the drug should be used as prescribed by a doctor.
Overdose
There is no information about cases of overdose.
In case of overdose, the symptoms mentioned in the "Adverse reactions" section may be aggravated.
In case of overdose, discontinue use of the drug, rinse the skin with plenty of warm water. Antihistamines may be used.
Adverse reactions
In some cases, skin irritation, burning, tingling or prickling sensation, increased itching, swelling, numbness of the skin, paresthesia, pain, erythema, eczema, allergic reactions, including skin rashes, swelling may occur. In such cases, the use of the ointment should be discontinued. In patients with scabies, itching may persist for 4 weeks after the end of treatment. This may be due to the presence of an allergic reaction to the dead remains of the scabies mite under the skin and does not indicate ineffectiveness of the treatment.
