Composition
active ingredients: 1 g of ointment contains: 166.70 mg of standardized bacterial culture suspension (containing 5.0 x 108 E. coli, killed and preserved in 3.3 mg of liquefied phenol), 2.50 mg of hydrocortisone;
excipients: lanolin, yellow soft paraffin.

Pharmacological properties
Pharmacodynamics
The drug stimulates local immune processes due to antigenic action, causing both nonspecific and specific immune reactions, resulting in increased local immune resistance. Accelerates and improves wound healing, has an anti-inflammatory effect, prevents reinfection and superinfection.
Pharmacokinetics
Due to sorption or transcytosis, components of the bacterial culture suspension act as antigens and initiate the process of local immunization, which, in turn, causes a protective immune response.
Indication
Topical treatment of symptoms of hemorrhoids, eczema and anal fissures.
Contraindication
Hypersensitivity to the active substance or to any component of the drug.
Specific skin conditions (tuberculosis, syphilis, gonorrhea) in the area of application, chickenpox, reaction to vaccination, fungal diseases, inflammatory diseases of the facial skin (oral dermatitis, rosacea), other bacterial, viral, parasitic infections at the site of application, ulcerative lesions and atrophic changes at the site of application, tumors of the anorectal area.
Interaction with other medicinal products and other types of interactions
When Posterisan Forte ointment is used simultaneously with other drugs containing corticosteroids (in the form of tablets, drops or injections), the therapeutic and side effects of corticosteroids may be enhanced.
Application features
Continue treatment until symptoms disappear, but not longer than 5-7 days.
When using any topical steroids, the possibility of systemic absorption should be kept in mind.
In case of fungal infections, a topical antifungal medication should be used simultaneously.
During treatment with Posterizan Forte ointment, the tensile strength of condoms may decrease due to the content of the yellow soft paraffin component, and thus this may negatively affect the quality of the condom.
Contains lanolin. May cause local skin reactions (e.g. contact dermatitis).
Use during pregnancy or breastfeeding
Pregnancy. Corticosteroids cross the placenta. There is currently no clear evidence of teratogenic effects in humans similar to those observed in animal studies. Effects on the fetus or newborn (in utero growth retardation, adrenocortical suppression) have been reported with systemic administration of high doses of corticosteroids.
Although there is limited information on the use of topical corticosteroids during pregnancy, due to their limited systemic absorption, the use of weak and moderately potent corticosteroids (class 1 and class 2), such as hydrocortisone, for short periods of time and on limited areas of skin is possible. The above-mentioned effects cannot be excluded with long-term use or application to large areas of skin. This should only be done if there are special indications.
Breastfeeding. Hydrocortisone should only be used by breastfeeding women for short-term treatment and on small areas of skin. Breastfeeding should be discontinued for long-term treatment or application to large areas of affected skin.

Method of administration and doses
Before use, it is necessary to carry out hygienic procedures of the anal area.
Adults should apply the ointment to the affected area of the skin and mucous membranes twice a day (morning and evening), if possible after defecation, and gently rub in with a finger.
To introduce the ointment into the anal canal and rectum, you can use the applicator with side holes, which is included in the package.
The duration of treatment is determined by the doctor, depending on the nature and course of the disease. Usually the duration of use is 5-7 days.
Children
Do not use on children.
Overdose
No cases of overdose or intoxication have been reported. Overdose causes suppression of the function of the hypothalamic-pituitary-adrenal system. In case of accidental oral administration of the ointment (e.g. by children), gastrointestinal disorders (abdominal pain, nausea) are possible.
If symptoms of hypercorticism occur, treatment should be discontinued.
Adverse reactions
In some cases, patients with hypersensitivity to the components of Posterisan Forte ointment may experience hypersensitivity reactions, including itching and burning, depigmentation, hypertrichosis, and dry skin.
May cause contact dermatitis (e.g. due to hypersensitivity to phenol).
With prolonged use of the drug in high doses, the following dermatological reactions are possible: dermatitis, eczema, delayed healing of wounds and cracks, development of secondary infection, atrophy of the mucous membrane, atrophy of the skin, adrenocortical suppression, dilation of capillaries (telangiectasia), the appearance of stretch marks on the skin (striae) and steroid acne.
