Reopoliglyukin solution for infusions bottle 200 ml




Pharmacological properties
10% colloidal solution of dextran (glucose polymer / glucose /) with a molecular weight of 30,000-40,000 with the addition of isotonic sodium chloride solution. Reopoliglyukin is a plasma-replacing solution that improves the rheological properties of blood, reduces its viscosity, helps restore blood flow in small capillaries, prevents and reduces aggregation of formed blood elements. With rapid transfusion of the drug, the volume of blood plasma can increase almost 2 times compared to the volume of the drug administered.
It is excreted from the body mainly in the urine (about 70% per day). The remaining amount enters the reticuloendothelial system, where it gradually breaks down to glucose. Reopoliglyukin is pyrogen-free.
Indication
It is used to improve capillary blood flow for the prevention and treatment of traumatic, surgical, toxic and burn shock; to improve arterial and venous blood circulation for the prevention and treatment of thrombosis, thrombophlebitis, endarteritis, Raynaud's disease; to add to the perfusion fluid in artificial blood circulation devices during heart surgery; to improve microcirculation and reduce the tendency to thrombosis in the transplant during vascular and plastic surgery.
Application
The doses and rate of administration of the drug are set individually.
In case of impaired capillary blood flow (various forms of shock), it is administered intravenously drip once at a dose of 10 ml/kg. The daily dose for adults and children is up to 15 ml/kg. Together with Reopoliglyukin, it is advisable to administer balanced crystalloid solutions in the amount necessary to replenish and maintain water and electrolyte balance (especially important in the treatment of patients with dehydration and after surgical interventions).
During cardiovascular and plastic surgeries, Reopoliglyukin is administered intravenously drip, immediately before the surgery - at a dose of 5-10 ml/kg (children - up to 10 ml/kg), during the surgery - 5 ml/kg (children - 10 ml/kg), after the surgery for 5-6 days - intravenously drip at the rate of 10 ml/kg per single injection (children under 2-3 years old - up to 10 ml/kg).
During operations with artificial blood circulation, Reopoliglyukin is added to the blood at the rate of 10-30 ml per 1 kg of body weight to fill the oxygenator pump; the concentration of the polymer with glucose in the perfusion solution should not exceed 3%. In the postoperative period, the doses of the drug are the same as in cases of impaired capillary blood flow.
Contraindication
Thrombocytopenia, kidney diseases accompanied by anuria, heart failure, conditions in which it is impossible to administer fluids in large volumes. Reopoliglyukin with 0.9% sodium chloride solution should not be administered in case of pathological changes in the kidneys, and Reopoliglyukin with 5% glucose solution - in case of carbohydrate metabolism disorders and especially in diabetes mellitus.
Side effects
As a rule, the drug causes an increase in diuresis, sometimes during treatment with rheopoliglyukin, a decrease in diuresis is observed, the urine becomes viscous, which may indicate dehydration of the patient's body. In this case, it is necessary to inject crystalloid solutions intravenously to replenish and maintain water and electrolyte balance. When reopoliglyukin is transfused, allergic reactions may develop. In these cases, calcium preparations, antihistamines, cardiovascular and other drugs are prescribed.
Special instructions
In case of impaired renal function or if it is necessary to limit the administration of sodium chloride, reopoliglyukin with 5% glucose solution is prescribed. In case of carbohydrate metabolism disorders and in other conditions when the administration of carbohydrates is contraindicated, reopoliglyukin with physiological sodium chloride solution is used. It is used only under the supervision of a physician.
