Sodium chloride 0.9% 500 ml No. 10

The solution for infusions "Sodium chloride isotonic solution 0.9% B. Braun" is recommended for use in the following indications:
sodium deficiency; loss of chlorine ions; for replenishing fluid and electrolytes in hypochloremic alkalosis; for short-term replenishment of intravascular fluid volume; hypotonic dehydration or isotonic dehydration; as a solvent for common electrolyte concentrates and drugs; for washing wounds, moistening tamponades and dressings (apply externally).Composition
Active ingredient: sodium chloride. : 1000 ml of solution contain 9.0 g sodium chloride Electrolytes: sodium - 154 mmol / l; chlorides - 154 mmol / l.
The excipient is water for injection.
Contraindication
It is forbidden to administer "Sodium Chloride Isotonic Solution 0.9% B. Brown" to patients with the following conditions:
hyperhydration; severe hypernatremia; severe hyperchloremia.Method of application
Adults: The dose is determined depending on the actual need for replenishment of water and electrolytes.
Maximum daily dose: up to 40 ml/kg body weight per day, which corresponds to 6 mmol of sodium per 1 kg of body weight.
Additional fluid losses (e.g. due to fever, diarrhea, vomiting, etc.) should be compensated depending on the volume and composition of the lost fluid.
In the treatment of acute fluid deficit, namely severe or life-threatening hypovolemic shock, high doses are allowed, for example by rapid infusion (under pressure).
In the treatment of sodium deficiency, the amount of sodium required to restore the plasma sodium level can be calculated using the formula: sodium requirement (mmol) = (desired - actual serum Na) × TBW, where TBW (total body water) is calculated as a proportion of body weight (this proportion is 0.6 in children, 0.6 and 0.5 in adult males and females, and 0.5 and 0.45 in elderly males and females, respectively).
The infusion rate depends on the individual patient's condition.
In general, the same dose should be used for elderly patients as for other adult patients, but special precautions should be taken when treating patients with diseases such as heart failure or kidney failure, which are often associated with old age.
When using the drug "Sodium Chloride Isotonic Solution 0.9% B. Braun" as a solvent, the dosage and infusion rate are determined mainly based on the characteristics and dosage regimen of the dissolved agent.
The volume of solution for washing wounds or moistening dressings is determined based on existing needs.
Method of administration: Intravenous administration or flushing and hydration. When performing rapid infusion using a solution in polyethylene bottles, air must be completely removed from the bottle and infusion system before starting the infusion.
Children. The dose should be determined depending on the individual need for replenishment of water and electrolytes, as well as the age, body weight and clinical condition of the patient. In case of severe dehydration, a bolus injection of 20 ml/kg body weight is recommended during the first hour of treatment. When administering this solution, the daily fluid intake should be taken into account.
Application features
Pregnant women
Data on the use of the drug "Sodium chloride solution isotonic 0.9% B. Braun" during pregnancy are limited. These data do not confirm the presence of direct or indirect harmful effects of sodium chloride 9 mg/ml, which would indicate reproductive toxicity. Since the concentration of sodium and chloride corresponds to the concentration of these substances in the human body, no harmful effects are expected when using the drug according to indications. Therefore, "Sodium chloride solution isotonic 0.9% B. Braun" can be used according to indications. However, precautions should be taken in the presence of eclampsia. If necessary, this drug can be used during breastfeeding.
Drivers
This medicine has no or negligible influence on the ability to drive or use machines.
Overdose
Symptoms
Overdose of the drug "Sodium chloride solution isotonic 0.9% B. Braun" can lead to hypernatremia, hyperchloremia, hyperhydration, acute volume overload, edema, serum hyperosmolarity and hyperchloremic acidosis. A rapid increase in serum sodium levels in patients with chronic hyponatremia can lead to the development of osmotic demyelination syndrome. The first signs of overdose may include thirst, confusion, sweating, headache, weakness, drowsiness and tachycardia. In case of severe hypernatremia, hypertension or hypotension, respiratory failure or coma may occur.
Treatment
Depending on the severity of the disorders: immediate cessation of the infusion, administration of diuretics with monitoring of serum electrolyte levels, correction of electrolyte levels and acid-base disturbances. In case of severe overdose or in case of oliguria or anuria, dialysis may be necessary.
Side effects
When using the drug according to the instructions, no adverse reactions have been reported. When conducting massive infusions, the development of chloride acidosis is possible.
Storage conditions
Store at a temperature not exceeding 25 °C, out of the reach of children. Do not freeze.
Shelf life - 3 years.
