Pharmacological properties
A cardiac glycoside that has a positive effect, as well as a negative chrono- and dromotropic effect.
In heart failure, it increases stroke volume and cardiac output, improves ventricular emptying, which leads to a decrease in heart size and a decrease in myocardial oxygen demand.
The effect of the drug appears 2-10 minutes after saturation is reached. The duration of action of Strophantin-G is from 1 to 3 days.
Indication
Acute cardiovascular failure, chronic heart failure stage III-IV according to NYHA, supraventricular tachycardia, atrial fibrillation.
Application
In / in slowly. Maximum single dose - 0.25 mg, daily - 1 mg. The drug is administered in low doses of 0.1-0.15 mg with an interval of 30 minutes to 2 hours. At an average rate of digitalization during the saturation period, 0.25 mg is usually administered 2 times a day with an interval of 12 hours. The duration of the saturation period is on average 2 days. The maintenance dose of strophanthin-g, as a rule, does not exceed 0.25 mg/day.
Contraindication
Glycoside intoxication, ventricular tachycardia, AV block II and III degree, severe bradycardia, hypercalcemia, hypokalemia, isolated mitral stenosis, hypertrophic obstructive cardiomyopathy, carotid sinus syndrome, thoracic aortic aneurysm, WPW S-M, acute myocardial infarction.
Side effects
Allergic reactions are possible (rarely). Other side effects of strophanthin-g are mainly due to its overdose or the patient's hypersensitivity to cardiac glycosides.
Interactions
The simultaneous use of strophanthin-g with sympathomimetics, methylxanthines, reserpine and tricyclic antidepressants increases the risk of developing arrhythmia.
The concentration of Strophanthin-G in blood plasma increases with the simultaneous administration of quinidine, amiodarone, and captopril.
Diuretics, corticosteroids, insulin, and calcium supplements increase the risk of glycoside intoxication.
Overdose
Symptoms of overdose of strophanthin-g (glycoside intoxication) are diverse. From the cardiovascular system: arrhythmias, including bradycardia, AV block, ventricular tachycardia or extrasystole, ventricular fibrillation. From the digestive tract: anorexia, nausea, vomiting, diarrhea. From the central nervous system and sensory organs: headache, increased fatigue, dizziness, rarely - coloring of surrounding objects in green and yellow, flickering of flies before the eyes, decreased visual acuity, scotoma, macro- and micropsia; very rarely - confusion, syncope. If glycoside intoxication develops, the drug should be canceled; prescribe potassium preparations, parenteral administration of unitiol, symptomatic therapy.
