Synthomycin
(Synthomycin)
Composition:
Active ingredient: 1 g of liniment contains chloramphenicol (synthomycin) - 50 mg;
excipients: castor oil, emulsifier No. 1, sorbic acid, sodium carboxymethylcellulose, purified water;
excipients included in emulsifier No. 1: cetyl alcohol, stearyl alcohol.
Dosage form.
Liniment.
Main physicochemical properties: liniment white or white with a slightly yellowish tint, with a faint specific odor.
Pharmacotherapeutic group.
Antibiotics for topical use. Chloramphenicol. ATC code d06a x02.
Pharmacological properties.
The drug is active against strains of bacteria resistant to penicillin, streptomycin, sulfonamides; gram-positive (staphilococcus spp., streptococcus spp.) and gram-negative (neisseria gonorrhoeae, neisseria meningitides) cocci, many bacteria (escherichia coli, haemophilus influenzae, salmonella spp., shigella spp., klebsiella spp., serratia spp., proteus stuartii, yersinia), rickettsiae, spirochetes, pathogens of trachoma, psittacosis, inguinal lymphogranulomatosis, has antimicrobial and antibacterial (bacteriostatic) action. The mechanism of action is associated with a violation of the synthesis of proteins of microorganisms. The resistance of microorganisms to chloramphenicol develops relatively slowly.
Clinical characteristics.
Indication.
Local treatment of purulent-inflammatory skin lesions (carbuncles, boils), purulent wounds, trophic ulcers that do not heal for a long time, second- and third-degree burns, cracked nipples in women in labor.
Contraindication.
Individual hypersensitivity to the components of the drug. psoriasis, eczema, fungal skin diseases.
Interaction with other drugs and other types of interactions.
Simultaneous use of the drug with erythromycin, oleandomycin, nystatin and levorin increases the antibacterial activity of the liniment, and with benzylpenicillin salts - reduces it. The drug is incompatible with sulfonamides.
The drug is incompatible with cytostatics, diphenylamines, barbiturates and pyrazolone derivatives, and ethanol.
Application features.
Do not allow the liniment to get into the eyes. Do not allow uncontrolled treatment with the drug in pediatric practice, use only as prescribed by a doctor.
Antibacterial agents for external use can lead to skin sensitization, which is accompanied by the development of hypersensitivity reactions in the future (when these drugs are prescribed externally or in the form of a dosage form for systemic action).
Use during pregnancy or breastfeeding.
Since the pharmacokinetics of synthomycin have not been studied, the drug should be used with caution during pregnancy and only when, in the opinion of the doctor, the expected benefit to the mother outweighs the potential risk to the fetus.
During the treatment of cracked nipples in postpartum women, breastfeeding does not need to be stopped.
Before breastfeeding, any remaining product should be removed from the nipple and surrounding skin with a clean cloth, and the nipple should be washed thoroughly and gently.
The ability to influence the reaction speed when driving or working with other mechanisms.

Method of administration and doses.
Gauze swabs are soaked with the drug or a thin layer is applied directly to the affected areas. A regular bandage is applied on top, it is possible with parchment or compress paper. Bandages are carried out depending on the indications after 1-3 days, sometimes - after 4-5 days. The duration of treatment is determined by the doctor individually.
In case of cracked nipples in women in labor, after each feeding, the nipples are washed with a 0.25% solution of ammonia, then a sterile napkin covered with a layer of liniment is applied to the crack. This quickly relieves sharp pain and 2-5 days after the start of treatment, the cracks heal.
Children.
Not prescribed to children in the first 4 weeks of life.
Overdose.
Symptoms: possible increase in side effects of the drug.
Treatment: symptomatic.

Allergic reactions are possible (including skin rashes, itching, burning sensation, hyperemia, edema).
